I searched but couldn't find anything to do with this.

When I enable the speedhack, what does the sleeptime do?

What do you mean by wait time?
If I don't want any wait time, should I set it at 0?

What does the wait time do?

It's more a internal value, usually automatically updated when you change speed, but you can change it.

The speedhack works by controlling the time in the game by pausing the clock, and then continueing the clock after a few milliseconds with a specific ammount of time.
The time the click is halted is defined by sleeptime.
A very low sleeptime will cause the game to run very slow since all cpu time is used up by the time ticker, but runs smooth, if you set it high the game speed multiplication will be accurate, set it too high, and the game will play shocking. So a sleeptime that's not too slow and not too high will give best performance for the speedhack
