Both options will leave you with a sense of remorse that you should have chosen the other option, so it's really annoying and doesn't matter, it doesn't affect anything in the game. The only thing that changes is the conversation you and your cousin have in the car on the drive back.

I let him live like 5 missions ago. I felt it was funnier to leave him stranded in a country he didn't know existed.
