Size 5.
No dynamics at all.

| hmmm.. this may be a stupid question, but are you sure the "Brush" is selected ? because Pencil doesnt stimulate pressure. whats your PS version?

| Try enabling Other Dynamics, then set the Control for Opacity Jitter (0%) and Flow Jitter (0%) to Pen Pressure.

Checking Shape Dynamics and setting the Control to Pen Pressure on Size Jitter (0%) also worked and produced better results for me. I'm not entirely sure if it works for anyone else. Why not try it out yourself and see if it works?
