Say the year is 2009

If someone says last year that means a year before right?

2009 - 1 = 2008

Say there are 20 questions on a test

Then if someone says there on the last test question that means there on the 20th question right?

Why is this so?

Because you could either say its the last question or the 20th question. Assuming the person would realize the 20th question was the last one.
