Text, any image you want (i used a sig), right click image layer, clipping mask, done.

Add text affects such as drop shadow, stroke, etc.

Guess: Get a font that you can clear out the color inside (possibly make it transparent some how?) and put a picture inside, make 30, upload to imagerotate, ??? ,profit.
