Found this issue - gigantic icon in Volume Mixer (see attach) Cool

Of course this is because app icon 128x128x32. Changing main icon to 32x32x32 solve this issue.

Steps to reproduce
1. Open CE
2. Add any address
3. Assign hotkey and Activate Sound - Speak Text - Any Text
4. Use hotkey to speak text. Windows register app with sound output.
5. Windows -> Taskbar -> Notification area -> Sound Icon (Default) -> Open volume mixer.
