He's actually collecting the IP addresses of everybody who's loaded the image. It might not look nice, but it's superior in terms of data collecting.

Using PHP then I'm assuming. GD library to generate an image with text, and the $_SERVER['REMOTE_ADDR'] to get the IP address of whoever is visiting.

MySQL Database. I had one from a LONG time ago. Simply deleted the 5000~ish IP's I captured, and reused it.

Would advice to serialize an array, and put that in a text file. Reduces the load, and you can do search in the array by using 'in_array()':
