find out what 3d system it uses (opengl or direct3d) and then hook the renderer.
e.g at endscene add a last ID3DPrite call to render a small crosshair texture
Alternatively, get some transparent tape, put a piece of paper in the form of the crosshair you want, and stick it to the center of your screen

OMG! I lol'd so hard at this. I should definitely try this! But I can't really help here, never played this game. I was hoping to, in fact, find some clues on how to write an overlay program... I am quite new to all this C++ stuff as well as to all of this programming.
Though I can give a basic advice. Want to learn C++ and try it right away on a game: Source is your Ultimate answer. Very easy to understand and integrate and with AMAZING wiki with code examples and stuff. I use it to learn C right now and and do it relatively speaking very successfully.
Also this forum is not very friendly... "You have not yet been deemed worthy enough to have an avatar"??? WTH? LOL

Yeah.
And for the code from the book you have,
Compile the code and debug it. Don't use dev-c++ (Bloodshed), it's outdated.
Use MS VC++ compiler it's the bomb! or for an alternate, use code blocks.
Go to an assault cube modding forums, they could help.

The last thing I did was experiment with render targets. I tried to add a dynamic scope to weapons. It seems to work out on the crossbow with ironsights (like in smod), but its glitchy and I have no idea how to attach a scope to .357 LOL, though that was my initial idea.
Changing bullet types for weapons also is fun.
Only I don't like the testing, its annoying and long...
