One of the check boxes in the "Active" column is out of place, like having leading spaces.

it's by design.
You dragged that item into the 3th item in the list making it a child of that address.
To fix it you can just drag it out

also, if you rightclick on a "parent" entry you can change the ways it's handling the children. Like hiding them when not active, setting the same value when changed, and active/deactive the children when the parent is changed
