Self loathe is the reason why people inflict pain on themselves like he did. His body will destroy itself if in his mind he feels hatred that's directed inward. Most of the time it will be unconscious and the person won't realize that they are hurting themselves because they hate their own body.

Doesn't make it right, but it's actually fairly normal >_>
