I clean the inside of my every 2 months but, I use e-cigs and, needless to say where my tower is (can't be relocated) some of the exhaled vapor gets into the thing. Not as bad a cigarette smoke but it does attract a small amount of moisture and, therefor makes dust stick worse than if I were a 100% non smoker.

For most 2-3 time per year is good, more if you live in a dusty place (like on a dirt road where dust comes in open windows and cracks. More if you smoke too, because of the nicotine residue that is a dust magnet and glue.

every 3-4 months, reapply the thermal dust etc

Only when I can see visible dust layers inside. I've never cleaned inside my laptop though just a lightly damp cloth on the outside, keyboard, and screen.

It is good to keep the system clean always. The dust on the fan as well as in motherboard will definitely cause your system harm. Use a brush and clean it once per week. If the SMPS sparked off because of dust, then you need to waste your budget unnecessarily.
