Every now and then I just get a huge lag spike.
I've run a malware and virus scan, defragmented, disk cleanup, cleared cookies and flash cookies. I even cleared the prefetch folder.

any ideas?

Disable any browser add-ons or extensions or tool bars. Start to re-enable them 1 by 1. If the lag spikes appear without any enabled you can safely re-enable them all, those aren't the cause. If you get to 1 and it starts doing it, that is your problem.
