Im guessing that u applied bevel and emboss/contour to the square, make an shape with pentool, fill it with white and put down the opactiy and put the text, and apply contour or inner shadow, thats how u did it?

Contour And inner shadow on the text. Everything else is pretty much correct. Cept the square has a radial gradient from top right to bottom left.

There is no tut as far as I know, I just kinda experimented after finding the px range he used.

iTunes doesn't let me change the icon when i; right click>properties>change icon, is there any way around this?
