Some are updated daily and even more often than that. Some are updated once per week some monthly and some annually. I've seen a few that have never been updated.

If you are running your own server, you are more likely to keep visitors by updating your content. Unless your site is selling a service of some kind then you will not really need to update it that often.

If it is a community that you are trying to create then yes you should update it.

That's a negative ghost rider. It really depends on the websites content.

Take CE for example. If you go to the mainpage, there is very little that will change, probably when a new version is released but that might be it. The forums on the other hand are community driven so they change several times per hour. Googles mainpage only changes when they post a funny pic, otherwise it rarely changes.

On the opposite end news sites change quite often as they try to "scoop" everyone else. For those not familiar with the term scoop, it's slang and in this sense means to publish a story before anyone else. Bloggers do this as well.
